About the presentation:
While health providers might advise a healthier diet, many patients face a challenge of finding healthier, affordable foods. Through innovative 'food prescription' partnerships, patients have greater access and affordability to healthy, fresher foods. The program links health care delivery with a food system approach that benefit patients, farmers and providers - patients receive nutrition education and coupons for fruits and vegetables and farmers have an expanded or consistent customer base.
